Practice Assessment Document (PAD) for SNAs
The Practice Assessment Document is designed to support and guide SNAs towards successfully achieving the criteria set out in the Standards of proficiency for nursing associates and Standards for education and training (NMC 2018).
The PAD makes up a significant part of SNAs’ overall programme assessment. Continuous assessment is an integral aspect of assessment in practice and apprentices are expected to show evidence of consistent achievement. SNAs will work with and receive written feedback from a range of staff, including Practice Supervisors (PS) and Practice Assessors (PA). SNAs are required to reflect on their learning.

Timesheets
SNA
Student Nursing Associates (SNA) must provide evidence of the learning hours received, in order to meet the Nursing and Midwifery Council (NMC) requirements for registration and requirements for apprenticeship funding.
Without this evidence, continued attendance on the apprenticeship is at risk and apprentices cannot register with the NMC as a Nursing Associate on successful completion of the course.

TAP
Trainee Assistant Practitioner apprentices (TAP) must provide evidence of the learning hours received, in order to meet the requirements for apprenticeship funding. Without this evidence, continued attendance on the apprenticeship is at risk.

PLACEMENT PASSPORT
This provides key information for educators about apprentices’ mandatory training, DBS, vaccinations, emergency contact numbers and any developmental or learning needs that need to be considered while on placement. Once completed the Student Placement Passport 2023 should be kept in a safe place, in the apprentices’ PAD.
