Mentor Meeting Record  record of progress towards professional development targets Trainee, to be completed weekly
Lesson Plan Proforma  to be used by trainee to rationalise plan and evaluate formal observed lessons  Trainee; 8 times during the academic year 
Record of Teaching hours   to be used by trainee to record teaching hours accrued. To be kept up to date and reviewed at fixed points Trainee, and signed off by mentor
Mentor Observation Record  to be used by mentor to provide developmental feedback from formal observations ** supported by drop in session 1 as above ** Mentor; 4 times during academic year
