In your browser go to https://office.com

select F for Forms

If the F isn’t showing select the (+) and search for Forms

Select the drop down and pick New Form (the new quiz is similar but allows you to add answers and scores, plus add maths type question and equations etc.)

Add a title and description and some questions

Select Share, set the permissions on who can answer, then either grab the link, the qr code, the embed code or select email.

You can see the responses in the responses tab
