Integrated Care for the Older Person
This module is for health professionals working in primary care and the aim of the module is to provide a contextual foundation to integrated person centred care in complex health and social care systems for older people living with long term conditions and frailty

Entry Requirements
Applicants should be working in the UK as a healthcare professional for a minimum of 2 days per week.
Applicants at level 7 would normally possess a bachelor degree or equivalent or have evidence of the ability to study at postgraduate level
Assessments
A 3000 word critically reflective case study that exemplifies the student’s practice in leading the case management of a patient case.
