You have to complete a form and send evidence (in a freepost envelope which comes with the form) to support your application.
You can get help with the cost of:

  • NHS dental treatment
  • Eye test, contact lenses and glasses
  • Prescriptions
  • Hospital travel

…………and more.
There is set criteria, so do read the form carefully before you apply. Some of you may be exempt from paying any charge if you receive certain DWP Benefits or you have certain illnesses or disability.
