These pages are designed to guide you through the placement process BEFORE, DURING and AFTER your visit.

You MUST read ALL these pages to ensure you understand what you need to do.  After you have read AND UNDERSTOOD what is on each page, ALL STUDENTS need to complete the Self Declaration form BEFORE 1st November 2023.  NB If anything changes during the year, both from a health point of view or if you receive a police caution/sentence, you should IMMEDIATELY submit the following Form AND notify the Placement Coordinators within seven days.

PLEASE NOTE: Failure to complete the reading and then the form could lead to Fitness to Practise.

Students who have returned after Intermission MUST apply for a DBS IMMEDIATELY on their return to University.

Once you have completed all the Essential Criteria (checklist attached for your information), and we have received your DBS, completed Self Declaration form and, if you are in Year 1, Occupational Health clearance we will arrange a placement for you.

YEAR 4 STUDENTS ONLY MUST complete this form by the end of October 2023 about their Community Pharmacy placements.
